The Global initiative for chronic obstuctive lung disease(GOLD) given the report ,according to this,50% of the COPD cases returned to the baseline level after the 7 days of the treatment,but 14% of the COPD cases were not returned to the baseline level after 35 days of onset and some cases not even returned to the baseline level.
Above data shows that there should be an improvement in effectiveness of COPD outpatient management.
Readmission rates of COPD exacerbations,the risk of 30 day readmission is higher that is 21%.
tracer data performance improvement includes,training the patients during the hospital stay regarding the self care management skills specific to COPD, teaching breathing exercises etc.
