It is the information directly related to an individual that can suitably identifies the individual as the person to whome the service or treatment is offered.

Most commonly used patient identifier are patient full name and date of birth or medical id number.

In some cases if the patient is not wearing identification band,other identifiers may be needed to identify.

Patient identification must be done correctly at registration and admission time itself. Ensure all the documentation, medical notes and electronic system match with patient.
