An EMR ( Electronic Medical Record) refers to digital version of
patient’s data collected from clinician’s office. The data include
both standard data and clinical data.
This data are gathered by individual physicians and used for track
data overtime and to monitor vitals in patients in due time.
EHR :
An EHR ( Electronic Health Records) refers to data collected by
physicians who treated a patient in different hospitals. This is
more comprehensive than EMR and it include treatment history,
personal history also.
It includes the collection of health data from all the physicians
who treated a patient.
PHR :
An PHR ( Personal Health Records) are managed by patient and these
records are either set up by physicians, hospitals , or patients
and it is controlled by patients.
