Mrs. Poppy walks very slowly, but her gait remained steady. She had no problems ambulating the 250 feet to the nurses’ station and back. Mrs. Poppy denied any pain or SOB with ambulation. When you returned Mrs. Poppy to her room, she stated she needed to use the bathroom. She did not require any assistance with this ADL and only needed help in and out the bathroom. Mrs. Poppy reported having a large BM. She stops and washes her hands and then states she thinks she will take a nap. Mrs. Poppy states she does not need anything else at this time. You settle her in bed, hand her the call light, raise the side rails, and leave the room.
