Mrs. Poppy walks very slowly, but her gait remained steady. She had no problems ambulating the 250 feet to the nurses’ station and back. Mrs. Poppy denied any pain or SOB with ambulation. When you returned Mrs. Poppy to her room, she stated she needed to use the bathroom. She did not require any assistance with this ADL and only needed help in and out the bathroom. Mrs. Poppy reported having a large BM. She stops and washes her hands and then states she thinks she will take a nap. Mrs. Poppy states she does not need anything else at this time. You settle her in bed, hand her the call light, raise the side rails, and leave the room.

4.30 Pm; Mrs. Poppy was ambulated slowly in a steady gait for 250 feet distance with her safety belt and walker to nurses station with assistance and she has no complaints while walking. Even though she has required no assistance in activities in daily living, she need assistance while going to bathroom and was assisted to go to bathroom and she stated that she had large bowel movement. after she washed her hands,She has been settled to bed as she has no complaints. After raising side rails, she has been informed to press the call button if she needs any assistance
