Night shift: 2300-0700 h
At the start of the night shift at 11 pm, Patient C had intravenous (IV) fluids of dextrose in water (D5W) continuing to infuse at 100 mL/h until 12:30 am, when it was discontinued by the nurse as per the prescriber’s order.
Overnight, Patient C slept fairly well. However, at 3:45 am, Patient C experienced an episode of diarrhea measured for 135 mL and voided 230 mL. At 6:20 am, the morning oral medication was taken with 75 mL water and the surgical drain was emptied for 55 mL. Patient C did not experience any vomiting during this shift.
Calculate the I&O for this 2300-0700 h shift using the following I&O record.
Input and output chart defined as measurment and recording all fluid intake and output during a 24 hour period provides input output data about the client's fuild and electrolyte balance.
* Unit of measurement input and output is milliliter.
It help us to determine patient's fuild status:-
* If they hydrated
* If they are dehydrated
*There is any obstruction
*If there is any fuild overlord
Input & output chart of patient C during night shift 11:00pm to 7:00am
Fuild | Time | Input | Time | output |
Iv fuild Dextose (D5W) |
11pm | 100ml | 3:45am |
135ml stool 230 ml urine |
Water Intake |
6:20am | 75ml | 6:20am | 55ml |
Total input is | 175 ml input | Total output is | 420ml |
So the patient have lost excess water from the body than he intake .
