If the product or artifact the engineer made is faulty and all the other operators factors are perfectly ok , then even the most skillfull operators can cause harm. Only in this case can the engineer be deemed responsible. In all cases where the error occurs due to operators conditions and skills , the engineer is not responsible. Hence the answer is D)
