Two reasons why some U.S. citizens are without healthcare coverage despite public and private health insurance programs :
1. The premium to be paid for health insurance, have to be entirely born by the person. Even if an employer gives subsidies, certain low-income earners find it difficult to afford it.

2. Eligibility criterias are not met. E.g.- Medicaid being only for the elderly and certain disabilities, medicare eligibility on the basis of income and assets - with only the really poor qualifying sometimes.
