For example, is it income alone that explains differences in health between those with higher income and those with lower incomes? Or is it the differences between income levels juxtaposed with other factors, such as education?

Income alone definitely does not explain the differences in health between those with higher incomes and those with lower incomes. However, individuals with higher income are more likely to have the resources to be literate and educate themselves. Higher income to an extent corresponds with higher literacy rate. Thus, it can be assumed that individuals with higher income are more likely to be educated and thus, are capable to cater for medical and health needs. People with lower incomes would only be able to make their ends meet. Education and health are secondary. Perhaps it is a luxury. There are of course other factors that correlate with health, however income is also positively correlated with health and other factors.
