percentage yield is calculated from actual yield/theoretical yield x 100 percent. The difference between theoretical yield and practical yield is due to following reasons; 1; the reactants are not pure ,they contain some impurities thats why some side reactions take place and we did not get desired amount of product. 2; During purification method some of the product may lost by evaporation,distillation,filtration,some remain inside the vessel. 3; some reaction need energy to proceed.if the energy is removed before completion of reaction,then yield will be less than expected. 4;improper measurements also leads ti less yield.
