When solution in hot condition or excited state and don't allow to cool the complete reaction is not happening as product formed are not stabilize in excited state . For a reaction simple requrement is activation energy, when reaction in hot or excited than activation energy of reactions and product both high so complicated product not formed in reaction. Yield will lower.

b)

When hot water added imiditately to ice, temperature of ice increase suddenly and melt immediate.

You will get a normal temperature water from ice so it is not effective process so lose of ice happened.
