Why is HIV almost never listed as the cause of death in AIDS patients?

HIV virus affects the T-helper cells and hence affects both the humoral and cell mediated immunity. Due to this the body's immune system will be lowered and hence there is a chance for secondary infections to occur. But the HIV never causes any kind of pathogenic effect on any organ system. Hence due to the secondary infections which are caused due to the lowering of the immune system, the body's various organs get pathogenic affect by the secondary infections and hence lead to the organ damage and collapse which causes the death of the host. Hence the patients affected with HIV never listed as the cause of death in AIDS persons.
