A man complains about a shortness of breath and a cough. He has stabbing chest pain with inhalation and has coughed up blood. His X-Ray are cloudy on the left side of his lung and shows an enlarged heart. He has a pH of 7.25, heart rate/respiratory rate/ body temperature/pulse ox are all above the average range. Upon physical examination his lip are bluish. What is his possible diagnosis pneumonia, heart attack, or emphysema?

The possible diagnosis is heart attack because:

1. Shortness of breath and cough can occur in all the three conditions.

2. Stabbing pain is typical of heart attack. On inspiration as the lung expands it compresses the heart so the pain agrivates.

3 . Coughing of blood is due to the reason that the heart is not able to pump blood which cases backflow of blood in pulmonary circulation , leading to pulmonary congestion (causing shortness of breath), rupture of pulmonary vasculature leading to hemoptysis .

4. Enlarged heart is due to en enlargement of heart chambers to accommodate the extra blood.

5. Blue lips is due to inability of heart to pump blood to peripheral organs,As cyanosis is the sign of decrease oxygen supply.

All these points suggest cardiac pathology and from the above options there is only one cardiac pathology that is heart attack
