After arising from ascending aorta, the left coronary artery runs forward and to the left brtween the pulmonary trunk and the left auricle to divide into AIVA and circumflex artery. the AIVA runs down in the anterior interventricular groove to hte apex of the heart. It then passes posteriorly around the apex of the heart to enter the posterior interventricular groove to terminate by anastomosing with the posterior interventricular artery - a branch of right coronary artery.

Great cardiac vein accompanies AIVA and circumflex arteries to join the left end of coronary sinus.

Locations receiving blood from AIVA and sending blood into great cardiac vein are :

  • anterior part of interventricular septum
  • greater part of left ventricle
  • part of right ventricle
  • part of bundle branch of His
