In dilated cardiac myopathy the heart chambers are enlarged so that the ejection fraction is less in this disease condition. As the heart chambers are enlarged the blood remaining in the heart after the contraction increases. The ventricular filling too takes place slowly. So inorder to compensate that the heart rate increases as the body needs enough supply of blood for the oxygen and nutrients. In increasing the heart rate the blood supply is balanced.
In hypertropic cardiac myopathy the heart rate is elevated as the heart walls are thickened and the ventricles may be obstructed to pump enough blood. So the heart rate will increase as the preload decreases. Inorder to pump the blood the heart rate is increased so as to compensate the supply. The heart rate of HCM appears to be more irregular.
