70 year old male admitted 1 day history of headache
followed by a grand Male seizure. during seizure he sustained a
fracture of the neck and the left humerus. He was admitted feeling
a bit woozy on admission his pulse was 90 beats per minute. His
blood pressure was 100/60. the next day the patient was unconscious
and his blood pressure dropped to 80/40. on the ward round the
consultant could hear crepitations at the bases of both lungs. More
biochemical tests showed a urea of 15.7, creatine followed of 246,
and a potassium. of 5.2. The next day the patients blood pressure
had dropped to 75/35. his white cell count was 9.8, and his
haemoglobin was 8.5. despite further emergency treatment, the
patient deteriorated and died.
Past medical history:
1967-Duodenal ulcer
1967-Cholecystectomy
1980-Grand Male seizure
1981-pagets disease of right femur
1886-thyroidectomy
1987-grand Mal seizure
1993- TURP for prostatic carcinoma
1993- left hip replacement
What was the patients cause of death?
what were the signs and symptoms?
Patient's creatinine level is much higher than the normal level of creatinin( 0.6-1.2 mg/dl).
Which indicates that pt is suffering from renal failure. Examining his history we can see that there a thyroidectomy, so thyroid hormone levels are null in this body.
Now we know that thyroid hormone dysfunction may alter kidney function via direct renal effects & systemic haemodymic effects
On the other hand continue lowering of blood pressure also indicates improper clearing of waste products from our body.
So my presumption is that this death is due to acture kidney failure due to thyroidectomy.
