A white, 38-year old man with no family history was admitted to the hospital for extreme and abnormal weakness in his hands. He has a history of weakness and muscle atrophy in his right hand for 2 years, noticing the same in his left soon after. For the next 6 months, he had noticed a thinning of the arms (predominantly in his right and his right thigh.
After a general examination, it was observed that his right shoulder appeared flatter and that muscles in his hands had wasted away. Uncontrollable twitching in his tongue was also observed. He had significant wasting of muscles in his hands and arms and right thigh. After testing his reflexes, it was noted that all four limbs responded in an exaggerated manner and shook especially when flexing his deltoid, pectoral, and quadriceps muscles. All other motor, sensory, and cerebellar examinations were within normal limits.
A MRI was ordered and found the brain and spine to be normal.
