Esophageal varices develop due to portal hypertension (portal pressure > 10 mm Hg).

• Lower esophagus is drained by left gastric vein, which drain into portal vein. Increase in portal vein pressure lead to opening of collateral near lower esophagus, this lead to development of varices

Dnager - risk of visceral bleed, which usually presents aa upper GI bleed.
