Jim was burned severely in July 4th fireworks. He had 3rd degree burn and he was administered lot of fluid during the treatment process. He also had loss of muscles in certain area. What clinical effects would low blood pressure have on the kidneys of a burn patient? How would loss of muscles affect the kidney?
Due to severe burn, the patient has a low glomerular hydrostatic pressure associated with loss of water as a result of burns over the skin, which resulted in a decreased glomerular filtration rate. The condition is very harmful and may lead to failure of nephron to regulate countercurrent flow. This condition may be followed by anuria.
The condition becomes more critical when the muscle is burned and the myoglobin present in the blood exceeds its maximum transport leading to the clogging and plugging of renal tubules leading to nephornal damage and death. The large volume of parenteral fluid is necessary to administer in order to regulate the volume of blood, a high glomerular filtration to avoid the tragedy.
