A 38-year-old woman is concerned about her future potential to have children. She currently works in a demanding job and doesn’t have a partner with whom she would like to have a baby. She has never been pregnant in the past. She is considering whether she should attempt to conceive using donor sperm, but would rather delay this if feasible. She books a private consultation with a fertility specialist to discuss these concerns. The specialist recommends measuring her serum anti-Mullerian hormone level. 1) why do you think that this test is proposed? (tell me the pathway) 2) What advice could she be given about her fecundity?

Answer #1

1. Anti- mullerian test is conducted to find the quantity of oocyte present in the female ovary. Anti mullerian hormone is produced by the follicles in the ovary. These follicles are formed at the time of birth and most of them gets degenerated at the time of puberty. Still the follicles gets defeated with the increase in age. This hormone produced by the granulosa cells of the ovarian follicle determines still how much amount of follicles are remaining for maturation.

2. The woman much aged. Even at the time of 30 years the degenation of the follicles occurs and hormonal balance changes. She is at the age of 38 and thus there is less chance to concieve. If the anti mullerian hormone level is good enough now she can utilise the IVF technoly to get concieved. But not only that. The hormone level is also an important factor if we take her age into consideration.
