Vasopressin or ADH acts on the V2R(vasopressin 2 receptor) which is a Gs type causes increse in cGMP which phosphorylates aquaporins and facilitates their movement towards cell membrane of medullary collecting duct and causes water reabsorbtion.
Na+ is also reabsorbed from collecting duct with the help of ENaC channels (which is activated when aldosterone binds to mineralocorticoid receptor and forms ligand receptor complex which moves towards nucleus and causes release of transcription factors and they activate ENaC).This Na+ reabsorbtion causes depolarisation of epithelial cell of collecting duct which activates H+ATPase and ROMK channels leads to excretion of H+ and K+ ions respectively in urine.
