The chronic inflammation in case of tuberculosis represents granulomatosis in which a granuloma contains immune cells of blood derived and epithelioid cells (differentiated) macrophages surrounded by T-lymphocytes. However caseous granulomas are typical of TB.

It is formed when cells of immune system wall off the toxic substances that could be harmful but unable eliminate them out. But necrosis is caused due to damage or dieing the tissue of a particular area due to toxic factors.
