Both health status and fitness level are important in determining the appropriate exercise intensity for an individual. Importance of health status is that it will determine whether the individual will be able to perform work and exercises efficiently. If the individual is in any diseased condition, he won't be able to give 100% of his efforts and the results of the doing exercise will not be seen. Moreover, doing exercise in diseased state will lead to deterioration of health instead of improving it.

Fitness level is important in determining exercise intensity because you can't assign a heavy intensity workout to an individual whose body is not fit for that. Strength of the exercise has to determined according to the fitness of the individual and should be gradually increased. This will give time for body to adapt and results will be best observed.
