A director of the company was at one of the company’s construction sites when an employee complained to her that he did not have a safety helmet. The director told the employee to carry on working even though she was aware of the risk of death or serious injury by working without a helmet. Later that day the employee was killed when an iron bar fell onto his head as he was working. He would have survived if he had been wearing a safety helmet.
