I would suggest bleach in the washer after it has filled and begun the washing cycle. This will keep it from being on only one area. Also, hot water works best and it doesn't harm cotton. If you're worried that the jacket will shrink, hang it to dry.

If it's already been washed and dried it's a bigger problem. I've had fairly good luck with a mix I came up with. I put one of the sink and tub cleaners like Ajax or Comet into a dish and add just enough water to make a paste. I use a small brush to apply the mixture to the item in the area that's stained and let it sit for several hours. Then just launder it along with other clothes.
